1. Applicant should be the resident of Haryana.

2. The income of the parents/guardian should not exceed Rs.1000/- per month. If the income exceeds Rs.1000/- per month then Rs.50/- per month will be charged as maintenance charges.

What are the facilities provided by the Training Centre for the Adult Blind?

HOME FOR AGED & INFIRM, REWARI

Home for Aged and Infirm, Rewari provides maintenance, food, clothing, boarding, medical and recreational facilities free of cost to aged and infirm persons. Rs.50/- per month as pocket money to each aged and infirm person is also provided.

GOVT. INSTITUTE FOR THE BLIND PANIPAT

The State Government is running an Institute for the Blinds in the age group of 6 to 18 years in the Panipat.

FINANCIAL ASSISTANCE TO DESTITUTE CHILDREN.

Application form for giving financial assistance to destitute children.

The State Govt. provides financial assistance to those orphan and destitute children who do not have parental support.

What is the eligibility criteria for availing the financial assistance?

1. The age of the children should be below 21 years.
2. The income of the applicant should not exceed Rs.10,000/- Per annum.
3. Financial assistance will be given to two children of a family.

How to apply and where for availing the financial assistance?
1. The application should be signed by Sarpanch/Numberdar and attested by a Gazetted Officer or reported by Incharge, Municipal Committee.
2. Birth certificate of the child issued by the School Authority or Gazetted Officer will beacceptable.
3. Rs.30/- pm as assistance, per child will be sent through appropriate medium.

GRANT-IN-AID

Application form for financial assistance under the grant-in-aid schemes.

Under the Grant-in-aid Scheme Department of Social Justice and Empowerment Department, Haryana provides financial assistance in the shape of grant-in-aid to the voluntary organizations working for the welfare of leprosy patients, drug addicts, mentally retarded children and deaf & dumb. Assistance is also provided for social security, education, food and clothing etc. of orphan/destitute/street children, delinquents, widows and destitute women and their dependents, aged persons and to provide artificial limbs to the physically handicapped.

HANDICAPPED PENSION SCHEME

The State Govt. is providing handicapped pension @ Rs.200/-pm per person. The blind, deaf & dumb are also eligible for this pension:-

1. Applicant should be below the age of 18 years.
2. There should be 70% or above disability.
3. The income of the applicant does not exceed Rs.10,000/-per annum.
4. The applicant should not be receiving Rs.200/-pm or higher pension from other sources.
5. Applicant should possess Handicapped Certificate issued by Chief Medical Officer of a Govt. Hospital.

When the Application form will be filled?

Forms of eligible persons are filled up every month and sanctioned pension will be provided from the month from which they are found eligible.

Application Forms should be attested by Sarpanch/ Numberdar or Nagarpalika Incharge.

When the Handicapped pension will be disbursed?

The pension will be disbursed by the Patwari in village & officials of Nagarpalika in urban areas on or before 7th of every month on fixed place.

SCHEME FOR THE WELFARE OF CHILDREN IN NEED OF CARE AND PROTECTION

Application for assistance for institutional services for welfare of destitute children

The State Government is giving 90% of their total expenditure to voluntary organisations who are working for education, training and welfare of destitute/orphan children upto the age of 18 years.

What is the eligibility criteria for getting assistance under this scheme?

The children are either orphan or the income of the parents/guardian is not more than Rs.500/- per month.

What are the facilities available under this Scheme?

Food, accommodation, education, medical and training facilities are provided free of cost under this scheme to destitute children.

OLD AGE PENSION

The State Govt. provides Rs.200/- PM per person old age pension to the senior citizen of 60 years or above.

What is the eligibility criteria for old Age Pension?

1. Husband & wife are eligible to get the pension separately.
2. The Wife of Ex-Service man is eligible for the pension.

1. Income tax payee.
2. Whose children are Class I or Class II Gazetted Officers in Govt./non-Govt. organizations.
3. Whose children are in professional field.
4. Persons who themselves or their children are MP/Ex-MP/MLA/Chairman of Board or
Corporation.

Who will sanction the Pension?

A Scrutiny Committee comprising of Revenue Officer/Dy Revenue Officer, Doctor & DSWO has been constituted in every Distt./City to sanction the pension.

From where the pension will be enrolled?

For getting the pension sanctioned, interested persons may reach the pre- fixed place in their village/city for enrollment for which announcement will be made in advance.

When the pension will be disbursed?

The pension will be disbursed on or before 7th of every month by the Patwari in villages & employees of Municipal Committees in urban areas.

SCHOLARSHIP TO PHYSICALLY HANDICAPPED STUDENTS

The State Government is providing scholarships to encourage the physically handicapped students @ Rs. 100-Rs.500 pm according to the class in which they are studying.

PROCEDURE FOR AVAILING THE SCHOLARSHIP

What is eligibility criteria for scholarship to physically handicapped students?

1. Handicapped student must be the resident of Haryana State.
2. Person with disability of 40% or above
3. Parents/Gaurdian’s income should not exceed Rs.4,000/-pm (for 1st –8th class) & Rs.6,000/-pm (for 9th & higher class).
4. Failed student will not be given Scholarship for the same class again.
5. Applicant should possess the Medical certificate issued by Civil Surgeon/Medical Officer/Chief Medical Officer.
6. Applicant should possess the Income certificate issued by Tehsildar/Naib-Tehsildar.

How to apply?

Application forms will be available with concerned DSWO. and the filled up forms shall be sent through headmaster of school to the concerned DSWO every year.

From where the scholarship money will be received?

Scholarship will be sent by the concerned DSWO through Bank Draft/ Bank to the headmaster of the school.

SCHEME FOR THE WELFARE OF STREET CHILDREN

The State Government is providing grant-in-aid to Non-Government Organisations who are working in the field of welfare of street children on the recommendation of concerned District Social Welfare Officer or Deputy Commissioner. Under this Scheme street children means children who live on the streets/slums/footpath and are engaged in various activities like rag picking, petty vending, shoe-shining, coolies, helpers, cleaners, rickshaw pullers etc. Children who are employed in wayside tea-stalls, restaurants, automobile garages, tyre and tube workshops, machine shops etc. will also be considered as street children.

What is the eligibility criteria for assistance to NGOs?

a) Registered under the Society Registration Act, 1860.
b) It shall have proven capability of work in the area of Child Welfare for three years.
c) It shall have a capacity for reaching out to at least 100 Street Children.
d) It shall be recognized aided or approved by at least one State Government/Union Territory Administration.
e) It shall have a properly constituted managing body with its powers, duties and responsibilities clearly defined and laid down in written constitution.
f) It shall not run for profit to any individual or a body of individuals.

What are the facilities provided by the NGOs under this scheme?

In addition to the grant-in-aid, Education, Counseling, Medical, Entertainment and Craft Training free of cost for the welfare of street children.

1. Young widows who have no means of income.
2. Woman whose husband is suffering form T.B., or mental disease, and is not medically fit to earn is eligible for admission in Homes for Widows and Destitute Women.

What are the facilities provided by Homes for Widows and Destitute Women?

1. Free Accommodation.
2. Rs.200/- as allowance per month for each family member. Rs.50/- per month as cloth allowance to each family.
3. Rs.10,000/- as grant-in-aid for the marriage of their daughters.
4. Free education/Craft Training and employment in production centres.

2. Income of the applicant does not exceed Rs.10,000/- per annum.
3. Applicants who are getting pension Rs.200/-pm or above from other sources will not be eligible for widow pension.

1. Forms should be attested by Sarpanch/Numberdar/Municipal Commissioner.
2. Income report from respective Patwari.
3. Form will have to be submitted to Scrutiny Committee personally by the applicant on the fixed day of every month.
